They're between a rock and hard place.
They hate ACA because of whose name is attached to it and it's Medicaid expansion which most Republicans simply see as another entitlement program.
On the other hand they can't fully get rid of ACA either because most of their constituents are poor White folks that live in rural areas that generally always vote Republican, and they'd immediately lose their healthcare which would cause widespread outrage that they wouldn't likely forget when election season rolled back around.
That's why you see this half baked plan by Paul Ryan to try to placate to this crowd even though it will eventually increase the cost of healthcare and discourage people from coverage in the long term.
If they get sick the insurance and whatever they have in a health savings account won't be enough to cover their ailment, obviously this will save money in the long term which is all that matters to conservatives but many people will die especially when they find themselves with serious chronic ailments like cancer, heart disease, diabetes, etc.
Convince these folks the other side wants to take their guns, government is using their tax dollars to fund abortions in planned parenthood, their religious freedom is being threatened by Middle Easterners, and some Latin American immigrant that picks fruit and vegetables on a farm is taking their piece of the American dream you can get them to vote against their best interests everytime, and they won't see the 100 other ways you're cutting their throat.
