Let me say this about Iron Man 3. It did a good job in showing Tonys evolution as Iron Man. It isn't fun and games obviously. Tony at that point still hasn't got over the incident. That scene of his panic attack shows Tony ain't well. And that fueled the creation of Ultron. He still ain't well. He feels responsible for some kids death in Sokovia and he literally witnessed his parents murdered. Iron Man 3 gave us clues I believe that Iron Man dies defending civilians. Cause he is still carrying a big burden on his shoulders.
