On the cover of the new Suicide Squad Rebirth comic she is carrying a Joker's doll.

Also, in the most recent issue of Suicide Squad, #22, the whole issue is Harley Quinn talking to herself in her head. She is running through all these different scenario's that eventually turns into a nightmare and Joker's face showing up in plumes of smoke that she is running away from. Seems like she is trying to talk herself out of returning to him. As we all know she will go back though.

I think their break up happened in the New 52 "Death of The Family" story arc. It crossed over into Suicide Squad where harley was fucking with deadshot for the beginning issues until Joker showed up. He beat the shit out of her then she realized she had to fight him and stuff (after leaving with him for a number of issues). I dont know how or why but i am almost positive that is the story arc where they broke up. Afterwards "Joker's Daughter" joined the Suicide Squad and caused a rift with Harley.
